The Council on Aging for Southeastern VT, Inc. offers
supportive grants to eligible persons who are primary caregivers
to elders in Windham and Windsor Counties. The purpose of these
grants is to provide "respite," or a period of rest and
relief from the challenges of family care giving.

Two
types of grants are available for supporting family caregivers and
are described below. Both grants are intended to provide respite
to the unpaid primary caregiver of a client.

Based on:
- Diagnosis
of Alzheimer's disease or other memory disorder
- Income
level (up to 300%
of federal poverty level)

Based on:
- Client must qualify as needing supervision or assistance
with 2 Activities of Daily Living (bathing, dressing, mobility/
transfer ability or continence needs)

Assessment: A Council on Aging Case Manager can work with
clients and caregivers to assess the client and caregivers needs.
The Case Manager can explore with you all appropriate options and
available assistance.
