A Survey to Identify information and Resource Needs

The Council on Aging for Southeastern Vermont is grateful for your responses to the following questions. We hope to use the information you and other grandparents provide to develop a "picture" of the information and resource needs you have. The information provided by you will remain private and confidential, and will only be used in summary reports (combining all survey results in one report without identifying names or information).
